Sample Conditions of Teleworking Agreement for
JANE TELEWORKER
January 2003

The following conditions for teleworking are agreed upon by the employee and his or her supervisor: 

1. The address of the Alternate Work Site is:

8334 Northway Rd.
Greenbelt, MD 20771

2. Describe the area in which the work will be performed:

A separate study on the first floor will be used for office space. It contains a telephone line, desk, chair, and appropriate lighting.

3. The employee's work schedule will be as follows:

  • Monday 8:00 a.m. - 4:30 p.m. at work site
  • Tuesday 8:30 a.m. - 5:00 p.m. at alternate work site
  • Wednesday 8:00 a.m. - 4:30 p.m. at work site
  • Thursday 8:00 a.m. - 12:00 p.m. at alternate work site & 12:30 p.m. - 4:30 p.m. at work site
  • Friday 8:00 a.m. - 4:30 p.m. at work site

Specific details outlining an intermittent arrangement

4. Detailed inventory of equipment used in performance of official duties (Indicate if Government-owned or employee-provided)

  • 486 Dell Processor Tag # 12345 (government-owned)
  • Hewlett-Packard LaserJet 3 Printer Tag # 23456 (government-owned)
  • Radio Shack Fax (employee-provided)
  • Hayes Modem - no tag (government-owned)

5. Access is authorized to the following Information Technology Resources  (Databases Containing Classified Information May Not Be Accessed from the Alternative Work Site):

  • CSB Administrative Mainframe
  • 110 LAN
  • RAMIS personnel databases

6. The following arrangements are agreed upon for communication between the alternate work site and the primary work site:

GSFC will provide a calling card for official long-distance calls made from the alternate work site. The employee agrees to check phone mail and E-mail for messages on a regular basis and respond in a timely manner consistent with providing excellent customer service. The employee must be available for phone calls during his regularly scheduled working hours.

7. Assignments to be worked on at alternate work site and expected delivery dates:
