Appendix B: Progress Reports

The intern submits progress reports in memorandum form. Level I reports are due at the end of the third and fifth months and every three months thereafter if the training program is extended. Level II reports are due at the end of the third, ninth, and fifteenth months. The sample memorandum on the following page indicates the format and the type of information that should be included in the report. Each report should be one to two typed single-spaced pages. The intern's progress report is to be submitted to the supervisor and forwarded with the supervisor's progress report to the PIP Coordinator, Code 114.

The report should describe the work assignments, intern's specific responsibilities, significant work experiences, training or workshops attended, and other requirements accomplished for each reporting period. Orientation activities and lectures attended should be briefly described by including topic, date, and speaker.

The intern should also indicate any other developmental activities he or she would like to have to enhance career objectives.
